Job Summary
Conduct initial evaluations, administer tests and measures, and synthesize results to formulate diagnoses, goals, and treatment plans for a patient caseload. Execute musculoskeletal interventions including therapeutic exercise, joint mobilization, and modalities while providing education on ADLs, IADLs, and rehabilitation equipment. Record daily patient care data, progress reports, and discharge plans in the Prompt EMR, and oversee support staff including PTAs and aides. Collaborate with the interdisciplinary team, manage billing productivity, and promote community outreach through lectures. Requires a licensed Physical Therapist with a Bachelor's or Master's degree, valid CPR, and experience with EMR software. Full-time role with up to $7,500 sign-on bonus and $2,500 relocation bonus.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision

- Identifying need for consultations and/or referring the patient to another health care professional or health care services
- Initiating and completing discharge plan
- Providing education/training to patient and/or patient support system
- Training in ADLs/IADLs, therapeutic activities (e.g. transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration
- Providing interventions to patients that are safe, effective, and in compliance with JAG
- Musculoskeletal Interventions including therapeutic exercise, joint mobilization and modalities
- Providing data, note writing using Prompt EMR for all treatments provided
- Overseeing support staff of PTA, ATC, PT aides
- Attending annual JAG Billing and Coding seminar
- Promoting JAG in the community through lectures/presentations
