Job Summary
Evaluate assigned patients by gathering data from records, interviews, and observations. Develop and implement individualized treatment programs, instructing patients and caregivers on home exercises. Provide direct therapy services across inpatient, outpatient, and home health settings while coordinating with medical and nursing teams. Prepare documentation including evaluation results, treatment plans, and progress notes. Communicate effectively with physicians and interdisciplinary members to track patient status and needs. Oversee physical therapy assistants, techs, and students to ensure safe patient care and efficient departmental operations. Maintain knowledge of proper equipment use and attend care conferences as applicable.
Required Qualifications
- Active license in the state of practice upon hire
- Physical Therapist - Licensing Board
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
- Ability to work the hours specified
- Ability to perform each essential job function satisfactorily
