Job Summary
Assess, plan, and administer rehabilitative programs to restore residents' function, alleviate pain, and support recovery from injury or illness. Develop individualized Plans of Treatment based on evaluations, setting short- and long-term goals while collaborating with physicians, staff, and families. Implement treatment schedules through manual exercises, massage therapy, and equipment use, adjusting plans as resident progress is evaluated. Document care services and communicate progress clearly to residents, caregivers, and healthcare professionals. Evaluate mobility, strength, and prosthetic fit, recommending modifications as needed. Coordinate transition plans for discharge to in-home care. Requires 2–3 years of experience, a current state license, and CPR certification. Full-time or PRN roles with a pay range of $43.60 to $63.65 BOE.
Required Qualifications
- Graduate of an accredited therapy program
- 2–3 years of related experience, or an equivalent combination of education and experience
- Current Physical Therapist state license
- CPR certification
