Job Summary
Provide in-home and senior living-based physical therapy with one-on-one patient care to older adults in Chesterfield, MO, delivering evaluations, individualized plans of care, and evidenced-based treatment. Work across private residences and senior living communities, collaborating with an interdisciplinary team, documenting in FOX’s EMR, and incorporating telehealth when appropriate to extend access. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
- Driving to patient homes or having reliable transportation
