Job Summary
Respond to physicians' treatment orders and perform thorough patient evaluations to assess needs for mobility, cognitive, and functional limitations. Establish measurable short and long-term goals, develop individualized care plans, and progress patients to achieve maximum function by discharge. Demonstrate therapeutic techniques, apply safety considerations, and communicate with physicians, families, and supportive staff to coordinate care. Document evaluations, progress, and education activities thoroughly in the EPIC system within 24 hours and according to department policy. Oversee clinical performance of PTAs, aides, and students, and facilitate discharge planning including equipment recommendations and home health referrals. Participate in student affiliation programs and contribute to performance improvement data.
Required Qualifications
- Minimum Bachelor's of Science in Physical Therapy
- New York State Physical Therapy license and registration or a valid temporary license
- BLS certification through the American Heart Association
- A physical therapist must be able to evaluate and provide treatment interventions to patients
- Must be able to work in a fast-paced department
- Position requires ability to accurately and completely document in a timely manner, utilizing the electronic medical record system EPIC
- Must have excellent communication and customer service skills
