Job Summary
Evaluate patient function and performance to develop individualized plans of care, including long and short-term goals and intervention selection. Complete initial and ongoing comprehensive assessments as assigned while providing therapeutic treatments in accordance with the physician's certified plan. Participate in care plan development and evaluation in partnership with patients, representatives, and caregivers. Deliver one-on-one home visits with a 1:1 patient ratio to improve quality of life. Requires current Minnesota Physical Therapist license, five years of experience, CPR certification, and valid driver's license.
Required Qualifications
- Current license as a Physical Therapist in the state of Minnesota
- Two (2) years of experience as a Physical Therapist
- Valid driver's license
- Proof of auto insurance
- CPR certification
- Able to travel within geographic area serviced by office
Desired Qualifications
- Experience as a Physical Therapist that is relevant to the patient population assigned preferred
