Job Summary
Perform home health physical therapy evaluations, develop detailed care plans, and coordinate with physicians to ensure quality patient outcomes. Restore function and independence for individuals by supervising two Physical Therapy Assistants and reporting patient progress. Manage documentation and administrative tasks while maintaining performance standards for the team. This role requires a current unrestricted CA license, BLS/CPR certification, and a valid driver's license, with an anticipated territory in the Healdsburg, CA area.
Required Qualifications
- Current unrestricted CA Physical Therapist License
- Valid CA Driver's License
- Current BLS/CPR Certification
- Knowledge of local community resources
- Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care
- Knowledge of state and federal homecare regulatory guidelines
- Experience home health care
- Experience with high acuity patients
- Experience working with an interdisciplinary team
- Case Management fundamentals

Desired Qualifications
- 1+ year experience supervising PTAs in a HH environment
- Knowledge of state and federal homecare regulatory guidelines preferred
- Experience home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
