Job Summary
Provide Physical Therapy services in home-health settings, assisting physicians with evaluating patients’ functional status and developing the plan of care; deliver therapy within licensed scope, coordinate with the health-care team, observe responses to treatment, and prepare patient progress summaries every 60 days; set rehab goals and guide patients in therapeutic and self-care activities to improve function. Requires active Louisiana PT license, CPR, at least 1 year of PT experience, and reliable transportation to patient homes; home health experience preferred. Required Qualifications
- Licensed by the Louisiana Physical Therapy Board
- At least one year of experience as a physical therapist
- CPR certification
