Job Summary
Provide care to residents at the highest level as part of CareOne’s in-house rehab team. Collaborate daily with nursing and clinical staff to deliver rehabilitation services across short-term rehab, assisted living, LTACH, and long-term care settings. Maintain comprehensive clinical documentation and billing records, participate in in-service training, and may travel to other facilities on a limited basis. Hold a Master’s or Doctorate in Physical Therapy and an active PT license for the operating state; preferred experience in older adult rehabilitation and familiarity with governmental/regulatory guidelines.
Required Qualifications
- Master's or Doctorate Degree in Physical Therapy
- Current and Active Physical Therapist license within the state of operations
