Job Summary
Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence under physician orders and care plan. Responsibilities include assisting in evaluating function, developing and revising the plan of care, advising families and agency personnel, completing initial history and evaluations, performing skilled PT procedures, documenting progress and reports to physicians, instructing patients and families in home programs, participating in staff meetings, supervising PTA/HHA when assigned, completing OASIS assessments and related reconciliations, utilizing ICD-10 codes, and maintaining professional standards and confidentiality; requires licensure/certification appropriate to state practice and adherence to agency policies.
Required Qualifications
- Licensed by the state where practicing (if licensure applies)
- Eligibility through CAPTE-accredited physical therapist education program or substantially equivalent program
- Option to meet state exam requirements if applicable
- Possession of or eligibility for required licenses/certifications (per state)
- Two years experience preferred (implied by scope of responsibilities)
- Ability to exercise initiative and independent judgment
- Compliance with agency policies and confidentiality requirements
