Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to improve mobility and independence. Deliver therapy sessions to support wellness and functional independence, document effectively in FOX’s EMR, collaborate with an interdisciplinary team, educate patients and caregivers, and offer telehealth/virtual care when appropriate to extend access and flexibility. Flexible scheduling and in-home/senior living settings, with opportunities across in-home, senior living, and virtual care; H1B sponsorship available for qualified clinicians; new graduates eligible for mentoring program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
