Job Summary
In-home Physical Therapist role providing PT evaluations and personalized care plans to patients in Plainville, MA, with flexible part-time/per diem scheduling. Responsibilities include delivering skilled PT interventions to improve strength, balance, mobility, and safety; educating patients and families on exercises and home safety; collaborating with a multidisciplinary team; and documenting care using a mobile EMR. Must have MA PT license, DPT or MPT, CPR certification, and reliable transportation. Flexible schedule, mileage reimbursement, and local home visits offered.
Required Qualifications
- Active MA PT license
- DPT or MPT from an accredited program
- CPR certified
- 1+ year of experience preferred (new grads welcome)
- Reliable vehicle & valid driver’s license
