Job Summary
Assess patients and develop individualized treatment plans for home health care in Hereford and surrounding areas. Provide PT services in patients' homes to improve mobility, strength, balance, and daily function. Monitor patient progress and update treatment plans as needed while coordinating care with PTAs and other healthcare professionals. Complete accurate and timely documentation and maintain a valid Texas PT license with at least one year of experience. Own your device and internet connection for schedule access and charting. Weekly pay ranges from $65 to $75 per visit for this PRN role within a faith-based, locally owned agency offering ongoing training and employee recognition.
Required Qualifications
- Active, unrestricted Texas PT license
- At least 1 year of PT experience
- Current BLS/CPR certification
- Valid driver's license and reliable transportation required for travel between patient visits
- Own device and reliable internet connection required to access your schedule and complete charting
- Strong communication and organizational skills
- Comfortable working independently
