Job Summary
In-home physical therapy evaluations and development of personalized care plans to improve strength, balance, gait, and functional mobility; educate patients and families on home exercise programs and safe mobility; collaborate with BrightStar’s interdisciplinary team (nurses, OTs, HHAs, case managers); document patient progress and visit notes using a mobile EMR; ensure patient safety and quality outcomes aligned with BrightStar’s standards and compliance.
Required Qualifications
- Active and unrestricted Massachusetts Physical Therapist license
- Graduate of an accredited PT program (DPT or MPT)
- Minimum 1 year of PT experience preferred (new grads welcome with supervision)
- Reliable vehicle and valid driver’s license
- CPR certification
