Job Summary
Perform in-home PT evaluations and create personalized care plans. Provide skilled PT interventions to enhance strength, balance, mobility, and safety. Educate clients and families on exercises and home safety. Collaborate with a multidisciplinary care team. Document care using mobile-friendly EMR. VA: Licensed Physical Therapist in MA with a DPT or MPT, CPR certification, and a reliable vehicle; flexible, part-time/per diem schedule; local in-home visits in Swansea and surrounding areas; mileage reimbursement.
Required Qualifications
- Active MA PT license
- DPT or MPT from an accredited program
- CPR certified
- 1+ year of experience preferred (new grads welcome)
- Reliable vehicle & valid driver’s license
