Physical Therapist

Physical Therapist needed in an outpatient clinic at BreakThrough Physical Therapy. Responsible for evaluating and treating patients with musculoskeletal and neurological conditions, developing and implementing personalized care plans, performing manual therapy and therapeutic exercises, neuromuscular re-education, balance and gait training, and patient education. Document care in the electronic medical record, track outcomes, adjust plans based on patient goals, and collaborate with an interdisciplinary team to ensure high-quality, evidence-based care.
