Physical Therapist

Provide physician-prescribed physical therapy in a home-health setting, aiming to improve or minimize residual physical disabilities and return patients to their optimum functional level. Collaborate in patient-care planning with the team, direct and supervise personnel as required, conduct initial assessments, develop and modify care plans and maintenance programs, and perform reassessments every 30 days. Must hold a PT degree (Bachelor’s, Master’s, or Doctorate) and be licensed in the practicing state; one year of home-health experience and Medicare OASIS knowledge are preferred.
