Job Summary
Physical Therapist to join BoldAge PACE as part of an interdisciplinary team. Responsible for planning, organizing, and implementing PT services for PACE participants, including initial and ongoing assessments, creating treatment plans, and educating participants and families on home programs. Conduct home safety assessments and recommend equipment or home modifications as needed. Deliver therapy programs in center, clinic, contracted facilities, and participants’ homes; document evaluations and treatments in participant records; and collaborate with the IDT to develop comprehensive care plans. Train PACE staff on body mechanics, transfers, gait, and safe activities of daily living; participate in quality improvement and ongoing education; maintain licensure and certifications, and ensure privacy and OSHA safety guidelines are followed. Requires advanced degree in Physical Therapy, current state licensure, and driver’s license with auto insurance; two years of interdisciplinary team experience preferred; experience working with elderly populations preferred. Full-time, days, with location in Dayton, OH.»
Required Qualifications
- Advanced degree in Physical Therapy
- Current license to practice Physical Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
- Current driver’s license and proof of auto insurance
