Job Summary
Plan, organize, and implement physical therapy services for PACE participants, including initial and ongoing in-person assessments, adaptive equipment evaluations, and home safety modifications. Administer treatment programs consistent with recognized standards, measure effectiveness, and revise plans as needed. Educate participants and families on procedures and home programs, while training staff on body mechanics, transfers, and gait patterns. Collaborate with the interdisciplinary team to develop comprehensive care plans, participate in daily meetings, and coordinate 24-hour care delivery. Provide care in centers, clinics, contracted facilities, and participants' homes, maintaining accurate documentation and adhering to safety policies. Requires a current physical therapy license and two years of interdisciplinary team experience.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license
- Proof of auto insurance
Desired Qualifications
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting
- 1 year of experience working with a frail or elderly population
- Experience with a frail or elderly population (training will be provided upon hiring if not present)
