Job Summary
Physical Therapist to provide direct patient care in a home-care setting, evaluating functional needs and outcomes, designing and implementing a care plan, and coordinating with staff and specialists. Responsibilities include documenting evaluations and treatment goals, updating prescriptions/signatures as needed, directing support staff to achieve patient goals, and traveling within the branch/office coverage area (up to 50% travel). Requires graduation from an APTA-accredited program and an active PT license, with at least 1 year of healthcare facility experience. The role may involve irregular hours and on-call duties as applicable, and emphasizes leadership to staff and community impact.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
