Job Summary
Physical Therapist needed to provide direct patient care in home settings through Elara Caring. Responsibilities include evaluating patient needs, designing and implementing care plans, coordinating with the care team, documenting outcomes, and contributing to program growth in the home-health environment. Requires graduation from an APTA-approved PT program, an active PT license, and willingness to travel within the service area; strong emphasis on compassionate, team-based care and adherence to standards of care. Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
