Job Summary
Evaluate and treat patients recovering from injury, disease, or surgery in their home environment by establishing treatment plans, administering exercises and modalities, and educating families on mobility and equipment use. Manage diverse diagnoses and disabilities in accordance with physician plans, adapting interventions to individual age-related needs while prioritizing safety, infection control, and evidence-based practice. Facilitate patient self-care and participate in student clinical education as opportunities arise. Work schedules are determined by patient care needs and may include weekends and holidays; minimum scheduled roles are 0.5 FTE, while Per Diem positions require at least one shift per month.
Required Qualifications
- State of Wisconsin Driver's License
- State of Wisconsin Professional licensure for Physical Therapist
- Bachelor's degree in physical therapy
- A minimum of 1 year of recent experience in physical therapy
- CPR/BLS Certification
- Must be available for weekends and holidays
- Must be able to work 0.5 FTE or 1 shift/month (Per Diem)
