Job Summary
Perform skilled physical therapy visits to adult and geriatric clients in assisted living, independent living, and senior congregate facilities across Somerset, Middlesex, Monmouth, and Union Counties, NJ. Complete comprehensive evaluations, reassessments, and individualized treatment plans focusing on mobility, strength, balance, and gait. Implement skin integrity interventions and collaborate with nurses to coordinate care, while educating families on safety and disease management. Document all visits accurately in the electronic medical record system. Full-time or per diem status with a $10,000 sign-on bonus for full-time employees.
Required Qualifications
- Current New Jersey Physical Therapist (PT) license
- Minimum of one year of recent experience as a Physical Therapist
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
- Ability to work independently and effectively manage time
- Strong communication, organizational, and clinical assessment skills
