Job Summary
Perform skilled physical therapy visits to adult and geriatric clients in assisted living, independent living, and senior congregate facilities across Somerset, Middlesex, Monmouth, and Union Counties, NJ. Conduct comprehensive evaluations and reassessments of mobility, strength, balance, gait, and functional status to develop individualized treatment plans using evidence-based interventions such as therapeutic exercise and manual therapy. Collaborate with interdisciplinary team members to coordinate care, educate clients and families on safety strategies, and implement skin integrity interventions as appropriate. Complete timely documentation using BAYADA's electronic medical record system.
Required Qualifications
- Current New Jersey Physical Therapist (PT) license
- Minimum of one year of recent experience as a Physical Therapist
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
- Ability to work independently and effectively manage time
- Strong communication, organizational, and clinical assessment skills
