Job Summary
Make home visits to clients in designated geographic territories to perform diagnostic tests, measure mobility and strength, and assess safety. Implement integumentary protective interventions for wound repair and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise the Physical Therapy care plan while participating with nursing in the multidisciplinary team. Educate clients and family members on rehabilitative care and activities to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current New Jersey Physical Therapist license, one year of recent experience, and graduation from an accredited program. Base pay ranges from $70 to $80 per point depending on full-time or per diem status.
Required Qualifications
- A current New Jersey Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
