Job Summary
Conduct home health visits to adult and geriatric clients in the Virginia Beach to Chesapeake, VA area, performing diagnostic tests for mobility, strength, balance, and safety. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while educating clients and family members on rehabilitative activities. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Maintain a current VA Physical Therapist license and demonstrate strong interpersonal and problem-solving skills.
Required Qualifications
- A current VA Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR) preferred
