Job Summary
Conduct home visits to clients in designated territories across Somerset, Middlesex, and Monmouth Counties, performing diagnostic tests for mobility, strength, balance, and safety assessments. Implement individualized care plans featuring manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess and revise care plans while collaborating with nursing on multidisciplinary evaluations. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document all observations and interventions using a state-of-the-art touch pad tablet. Requires a current NJ Physical Therapist license, one year of recent experience, and proficiency with electronic medical records.
Required Qualifications
- A current NJ Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
