Job Summary
Conduct home health visits to clients in the San Manuel, Oracle, and Mammoth areas, performing diagnostic tests for mobility, strength, balance, and safety assessments. Implement individualized care plans featuring manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess and revise care plans while collaborating with nursing staff, and educate clients and family members on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current AZ Physical Therapist license, one year of recent clinical experience, and AP/AAMA approved graduation. Full-time role with travel incentives for the Tri-Cities.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current AZ Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- Prior experience with electronic medical records (EMR)
