Job Summary
Conduct home health visits in Summerville, SC, performing diagnostic tests for mobility, strength, and balance while implementing wound care interventions. Develop and revise individualized care plans featuring manual therapeutic exercises and gait training, then educate clients and families on rehabilitative activities. Accurately document all observations and evaluations using a touch pad tablet as part of the multidisciplinary care team. Requires a current SC license, one year of recent experience, and proficiency with electronic medical records.
Required Qualifications
- A current SC Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
