Job Summary
Conduct one-on-one home health visits for adult and geriatric clients throughout Hudson County, performing skilled physical therapy assessments of mobility, strength, balance, and gait. Develop and implement individualized treatment plans using evidence-based interventions, including therapeutic exercise, gait training, manual therapy, and patient education on safety and disease management. Collaborate with nurses and physicians to coordinate care, complete comprehensive evaluations and ongoing reassessments, and maintain accurate documentation in the electronic medical record system. Manage your own schedule while traveling throughout the assigned territory.
Required Qualifications
- Current New Jersey Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Strong communication, organizational, and clinical assessment skills
- Ability to work independently and effectively manage a flexible schedule
- Reliable transportation and ability to travel throughout the assigned territory
Desired Qualifications
- Home health experience preferred, but not required
- Experience with electronic medical records (EMR) preferred
