Job Summary
Perform skilled physical therapy visits to adult and geriatric clients in assisted living, independent living, and senior congregate facilities across Somerset, Middlesex, Monmouth, and Union Counties. Conduct comprehensive evaluations and ongoing reassessments of mobility, strength, balance, gait, transfers, and functional status. Develop and implement individualized treatment plans utilizing therapeutic exercise, gait training, and manual therapy while collaborating with interdisciplinary team members. Educate clients, families, and caregivers on safety strategies and disease management, and complete timely documentation in the electronic medical record system.
Required Qualifications
- Current New Jersey Physical Therapist (PT) license
- Minimum of one year of recent experience as a Physical Therapist
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
- Ability to work independently and effectively manage time
- Strong communication, organizational, and clinical assessment skills
