Job Summary
Perform diagnostic tests and measurements including mobility, range of joints, transfer status, and balance assessments. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and wound repair interventions. Continually assess and revise care plans while educating clients and families on rehabilitative activities to promote independent living. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Join a multi-disciplinary team providing skilled nursing and rehabilitative care to help clients remain at home.
Required Qualifications
- A current Physical Therapist - PT license in Arizona
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR)
