Physical Therapist

Provide skilled physical therapy services through home visits within designated territories, performing comprehensive evaluations of mobility, strength, balance, and functional abilities. Develop individualized treatment plans based on patient needs and physician orders, then implement evidence-based interventions including therapeutic exercise, gait training, and mobility activities. Collaborate with nursing and interdisciplinary team members to coordinate care, while educating patients and caregivers to promote safety and independence. Continuously assess patient progress to modify treatment plans and accurately document all evaluations and interventions using electronic medical records.
