Job Summary
Conduct home health visits for clients in Florence and Marion Counties, performing diagnostic tests on mobility, strength, balance, and ambulation. Implement individualized care plans featuring manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess and revise care plans while collaborating with nursing on multidisciplinary strategies. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document all observations and interventions using a touch pad tablet. Requires a current SC Physical Therapist license, one year of recent experience, and proficiency with electronic medical records.
Required Qualifications
- A current SC Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills
