Job Summary
Conduct one-on-one physical therapy visits to adult and geriatric clients in assisted living, independent living, and senior congregate facilities across Somerset, Middlesex, Monmouth, and Union Counties, NJ. Perform skilled assessments of mobility, strength, balance, and gait to develop and implement individualized treatment plans using evidence-based interventions. Collaborate with interdisciplinary team members to coordinate care, educate clients and families on safety strategies, and complete timely documentation in the electronic medical record system. Full-time or per diem status with a $10,000 sign-on bonus for full-time employees.
Required Qualifications
- Current New Jersey Physical Therapist (PT) license
- Minimum of one year of recent experience as a Physical Therapist
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
- Ability to work independently and effectively manage time
- Strong communication, organizational, and clinical assessment skills
