Job Summary
Conduct home visits to adult and geriatric clients in designated Jacksonville territories to perform diagnostic tests, mobility assessments, and safety evaluations. Implement integumentary protective interventions for wound repair and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing staff and educating clients or family members on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current FL Physical Therapist license, one year of recent experience, and graduation from an accredited program. Full-time schedules available.
Required Qualifications
- A current FL Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR) preferred
