Job Summary
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for patients in residential homes. Provide in-home therapy services focused on improving physical and cognitive functioning while monitoring, documenting, and reporting patient responses to treatment changes. Educate patients and caregivers on therapeutic exercises, safety, and adaptive equipment usage. Supervise Physical Therapy Assistants and Certified Nursing Assistants as appropriate. Complete required documentation, prepare clinical reports, and participate in team and agency development activities. Schedule Monday through Friday day shifts with one weekend every six weeks and two rotating holidays. No on-call requirements.
Required Qualifications
- Masters degree or higher in Physical Therapy
- Active and unrestricted Physical Therapist (PT) license in the State of Illinois
- Minimum of 1 year of experience in a healthcare setting
- Valid drivers license
- ability to travel locally
- Monday–Friday | Day shift
- One weekend every six weeks (Saturday & Sunday, scheduled visits only)
- Two holidays per year (one summer, one winter on a rotating basis)
- No on-call requirements
