Physical Therapist

Evaluate patients referred for home care services to determine appropriateness for physical therapy services and establish a home care plan including an assessment of service appropriateness. Collaborate with agency personnel regularly to ensure the agency meets patient needs, explain agency policies and services to patients and family members during initial assessments, and complete documentation accurately for timely submission. Promote ongoing communication with agency staff and assure customer satisfaction.
