Job Summary
Evaluate assigned patients by gathering data from records, interviews, and observations. Develop and implement individualized treatment programs, instructing patients and caregivers on home exercises. Provide direct therapy services across inpatient, outpatient, and home health settings while coordinating with medical, nursing, and rehabilitation teams. Prepare documentation including evaluations, treatment plans, and progress notes. Oversee physical therapy assistants, techs, and students to ensure safe patient care and smooth departmental operations. Maintain knowledge of proper equipment use and attend care conferences as applicable.
Required Qualifications
- Active license in the state of practice upon hire
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
- Ability to work the hours specified
- Ability to perform each essential job function satisfactorily
