Job Summary
Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Delivers care per PT plan of care, documents care, and collaborates with supervising PT. Travels to patients’ homes and facilities to deliver direct care, utilizing required durable medical equipment, and assists with activities of daily living as needed. Maintains patient progress records and participates in interdisciplinary meetings. Requires NY State PTA certification, an approved PTA program degree, and a minimum of two years of clinical experience; may require a driver’s license. Home-based care environment with travel to diverse settings and potential for lifting and assisting patients.
Required Qualifications
- New York State Certification as Physical Therapist Assistant
- Associate's Degree as Physical Therapist Assistant from a NYS approved program or equivalent
- Valid driver’s license may be required
- Minimum of two years experience in clinical patient care
