Job Summary
Optimize functional abilities and skills of patients in assisted living, independent living, and memory care settings by implementing treatment plans and supervising rehab techs. Assist in screening and initial evaluations, while maintaining accurate billing and documentation in compliance with state and federal guidelines. Establish effective interactions with patients, interdisciplinary teams, and families to ensure patient-centered care. Provide direct supervision during group and individual treatments, contribute to community patient care and utilization reviews, and make appropriate referrals to other rehabilitation team members. Maintain valid state licensure, adhere to infection control and environmental safety protocols, and travel between communities during scheduled hours. This role requires patient lifting of up to thirty pounds and the ability to work varying shifts, including weekends and holidays.
Required Qualifications
- Must be capable of lifting thirty (30) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Must maintain valid state license (where appropriate)
- Graduate of an APTA accredited school of physical therapy
- Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports
- Ability to work in a fast-paced, productive work environment
- Must conduct self in an ethical, legal, and responsible manner at all times
- Must adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct
- Attends all mandatory meetings, trainings, and assignments as delegated
