Job Summary
Optimize functional abilities of patients in Assisted Living, Independent Living, and Memory Care settings by implementing treatment plans, assisting in initial evaluations, and supervising Rehab Techs. Manage case responsibilities, conduct utilization reviews, and facilitate interdisciplinary conferences while maintaining accurate billing and documentation. Establish and maintain effective interactions with patients, families, and the healthcare team, ensuring compliance with state regulations, infection control protocols, and environmental safety standards. Perform patient lifting using appropriate biomechanical techniques and adhere to all Reliant Rehabilitation policies and codes of conduct.
Required Qualifications
- Must be able to lift thirty (30) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Attends all mandatory meetings, trainings, and assignments as delegated
- Must maintain valid state license (where appropriate)
- Graduate of an APTA accredited school of physical therapy
