Job Summary
Optimize functional abilities of patients in Assisted Living, Independent Living, and Memory Care settings by screening, evaluating, and implementing treatment plans under a licensed Physical Therapist's direction. Supervise Rehab Techs during group and individual treatments while managing case responsibilities, utilization reviews, and interdisciplinary communications. Ensure accurate billing, documentation, and compliance with state/federal regulations, infection control protocols, and patient privacy standards. Perform patient lifting up to 30 pounds and travel between communities as required. Maintain valid state licensure, attend mandatory trainings, and adhere to Reliant Rehabilitation's Code of Business Conduct.
Required Qualifications
- Must be able to lift thirty (30) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Attends all mandatory meetings, trainings, and assignments as delegated
- Must maintain valid state license (where appropriate)
- Must be able to read, write, speak, and comprehend in English
- Graduate of an APTA accredited school of physical therapy
