Job Summary
Optimize functional abilities and skills of patients in assisted living, independent living, and memory care settings by implementing treatment plans, screening patients, and supervising rehab techs. Maintain positive interactions with clients, interdisciplinary teams, and families while ensuring accurate billing, documentation, and compliance with state and federal regulations. Provide direct oversight of rehab techs during group and individual treatments, manage case responsibilities, and contribute to community care, utilization reviews, and family conferences. Travel between communities as needed and adhere to infection control, safety, and privacy protocols. Work varying shifts including weekends and holidays, requiring the ability to lift 30 pounds of dead weight.
Required Qualifications
- Must be able to lift thirty (30) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Attends all mandatory meetings, trainings, and assignments as delegated
- Must conduct self in an ethical, legal, and responsible manner at all times
- Must adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct
- Graduate of an APTA accredited school of physical therapy
Desired Qualifications
- Maintain valid state license (where appropriate)
