Job Summary
Implement treatment for assigned patients under the direction of a licensed physical therapist while optimizing functional abilities and skills in assisted living, independent living, and memory care settings. Assist in screening and initial evaluations, supervise rehab techs during group and individual treatments, and manage case responsibilities including utilization review and family conferences. Maintain accurate billing and documentation, ensure infection control and environmental safety compliance, and make referrals to the broader rehabilitation team. Travel between communities and work varying shifts including weekends and holidays.
Required Qualifications
- Must be able to lift thirty (30) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Must maintain valid state license (where appropriate)
- Must be able to read, write, speak, and comprehend in English
- Must be able to work in a fast-paced, productive work environment
- Graduate of an APTA accredited school of physical therapy
Desired Qualifications
- Prior to beginning patient care for Reliant Rehabilitation, a peer PT may complete a Competency Checklist
