Physical Therapist Assistant

Hybrid Quality Care Coordinator / Physical Therapist Assistant role combining hands-on PTA care with care-management responsibilities. Responsibilities include developing and updating comprehensive client care plans, supervising in-home care to ensure safety and functional mobility, coordinating with families and medical referral sources, conducting in-home supervisory visits for quality assurance, scheduling to ensure continuity of care, and providing office and team support.
