Job Summary
Provides physical therapy interventions under the supervision of a Physical Therapist (PT) and assists in monitoring patient progress, engaging in PT plan of care, and education for patients and caregivers. Travels to patients’ homes or facilities to deliver direct PT services, documents care per policy, and collaborates with the supervising therapist. Requires NYS PTA certification (or equivalent), an Associates degree, and a minimum of two years clinical experience; includes responsibilities such as utilizing durable medical equipment, coordinating with interdisciplinary teams, and adhering to agency policies. Required Qualifications
- New York State Certification as Physical Therapist Assistant Required
- Associate‘s Degree as Physical Therapist Assistant from a New York State approved program or equivalent
- Minimum of two years experience in clinical patient care
- Valid driver’s license may be required
- Bilingual skills may be required
- Basic PC skills preferred
