Job Summary
Deliver physical therapy interventions and patient/caregiver education under the supervision of a Physical Therapist, modifying treatment procedures per the PT plan of care. Monitor patient progress through observation and follow up with the supervising therapist, ensuring all care is documented and records are maintained per VNS Health policy. Manage assigned caseloads while providing services to patients at varying environments, including home visits and care facilities, using approved transportation and durable medical equipment. Assist with activities of daily living, positioning, and transferring patients, instructing family members on capabilities, and participate in interdisciplinary team meetings and caseload reviews.
Required Qualifications
- New York State Certification as Physical Therapist Assistant
- Valid driver's license
- Associate's Degree as Physical Therapist Assistant from a New York State approved program
- Minimum of two years experience in clinical patient care
