Job Summary
Carry out individualized treatment plans established by a licensed Physical Therapist to improve strength, balance, gait, endurance, and mobility for residents in a skilled nursing and post-acute care setting. Provide therapeutic interventions while tracking progress, communicating updates to the supervising Physical Therapist, and educating residents and caregivers on safe mobility techniques, home exercises, and fall prevention. Maintain accurate clinical documentation meeting facility and regulatory standards and collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and the interdisciplinary care team. Direct hire only; no agency placement.
Required Qualifications
- Active Maryland Physical Therapist Assistant (PTA) license
- Strong communication skills
- Strong teamwork skills
