Job Summary
Coordinate with clinical caregivers to ensure residents and patients achieve full recovery and timely return to activities under Signature Rehab's integrated care model. Assist licensed physical therapists in developing and implementing individualized rehabilitation plans for skilled nursing and rehab private suite residents. Support patient discharge planning and monitor progress toward functional goals within the Tri-Cities area.
Required Qualifications
- Graduate of accredited Physical Therapy Assistant program, culminating in a minimum of an Associate's Degree", "A Valid Physical Therapy Assistant license in the state where services are rendered
